If you are a Kansas author who has published a book(s) or an author of a book(s) thematically about or set in Kansas and would like for your name and information to be considered for the Kansas Center for the Book's Author "online" Database, please contact us by filling out and submitting the following form.

Contributors submitting for a "deceased" author please provide the required "submitter" info in the red box located near the middle of the form.

We request that you include your phone number and e-mail address that we might respond or contact you if there is a need. NOTE that phone numbers will NOT be placed in the public accessible database. E-mails, however, will appear unless otherwise requested.

Addresses, birthdates, e-mails & website addresses WILL BE INCLUDED on your personal database page attached to the website, UNLESS you request otherwise.
